Accessing or using this site and the information therein constitutes
acceptance of the terms and conditions below. Oral representations or
agreements not set forth in this agreement are not binding. This agreement
supersedes any preceding agreement between the parties relating to the services
of this site.

of your employee benefit plan and your relationship with MVP. The accuracy, authenticity,
security, and confidentiality of the information submitted to MVP is the sole responsibility
of the party submitting the information. The use of this Terms and Conditions of Employer
Use does not, in any manner whatsoever, supersede, replace, or supplement, any terms or
conditions contained in any written documentation disseminated by MVP in any other manner,
other than through this website. In the event of a conflict between these Terms and Conditions
of Employer Use, unless indicated otherwise, documents published and mailed to your office
shall control. In the event of conflict between these Terms and Conditions of Employer Use
and a members Employee Benefit Plan Description, or other evidence of the terms and conditions
of benefit coverage, the members Employee Benefit Plan Description or other evidence of the
terms and conditions of benefit coverage shall control.

By use of this site you expressly agree that you are, and will continue
to abide all terms and conditions regarding Member Eligibility, Employee
Benefit Plan Documents, and any other standards as communicated to you from
time to time by MVP.

and services referenced, may contain inaccuracies, typographical errors,
